Skilled nurse for assessment, instruction, and performance of
treatments.
- Psychiatric nurses for assessment and instruction of mental status,
disease process, medication regime response and side effects, behavioral
management, coping/ relaxation/stress management skills etc.
- Home Health Aide for personal care, assistance with ADL's, therapeutic treatments, reinforcement of therapies.
- Physical, Occupational, and Speech Therapies
- Social Workers for coping with grief and loss issues, development
of DPOA-HC, financial counseling, long-term planning and decision-making,
assistance with transfer to a different level of care, etc.
- Registered Dietician for assessment/instruction in special diets
or weight management.
- Before scheduled surgery we will:
-Perform pre-op teaching.
-Facilitate smooth transition from hospital to home for continuity
of care.
-Develop relationships with discharge planners.
-Promote emotional support for the entire family.
- For newly diagnosed or unstable Diabetes we will perform diabetic teaching with the latest techniques in
addition
-Insulin preparation and administration
-Blood glucose monitoring
-Care of feet
-Diet instruction
- For all acute disease processes including people who require
additions or increases in medications:
-Urinary Tract Infections
-Upper Respiratory Infections
-GI Bleed
-Strokes, TIAs
-Exacerbation of Chronic Illnesses
(osteoarthritis, rhematoid arthritis, Parkinson's, hypertension,
etc.)
- When a resident transfers from SNF to Assisted Living
-Promotes continuity of care
-Encourages progression toward greater independence
-Promotes wellness and restoration through teaching and assessment
- For residents who mismanage medication
- When there is a progressive illness but the resident may not
be ready for Hospice.
- For residents who require wound care as in stasis ulcers, decubitus
ulcers or Vitamin B12
injections.
- For people in the community wishing to enter your facility but
due to space limitations may
be able to do so. We can provide Home Care until admission is possible
- For residents experiencing late effects of fractures, strokes,
falls.
